Who is the person responsible for the term "C/C++"?

I highly value the term "C/C++" as a shibboleth to weed out resumes of programmers who have loaded them up with buzzwords for extra programming languages that they don't really know how to use.

In my experience, anyone who puts "C/C++" on her or his resume is likely not to know either one well. As Sergey Zubkov mentions, the conventions, techniques, and libraries employed by the two languages are so different in most cases that there's little overlap besides some syntactic similarity and some ABI compatibility.
